ACKNOWLEDGMENT OF STUDENT SERVICES
I understand that I am receiving beauty, esthetics, nail, or cosmetology services at Ascend Beauty
College (“the School”) performed by students currently enrolled in a state-regulated educational program.
I acknowledge that:
- Student providers are not licensed professionals
- All services are performed under the supervision of licensed instructors
- Services may take longer than in a traditional salon/spa setting
- Results may vary and are not guaranteed
- Adjustments or corrections may be part of the learning process
I voluntarily agree to receive services from a student provider.
I certify that I have disclosed all known medical conditions, medications, allergies, and sensitivities that may affect my service. I understand that withholding relevant information may increase the risk of adverse reactions.
ASSUMPTION OF RISK
I understand that beauty services involve inherent risks including but not limited to:
- Skin irritation
- Redness or swelling
- Allergic reactions
- Discomfort
- Dissatisfaction with cosmetic results
I voluntarily assume all known and unknown risks associated with services performed by a student provider.
RELEASE OF LIABILITY
To the fullest extent permitted by law, I hereby release and discharge Ascend Beauty College, its owners, instructors, employees, students, agents, and affiliates from any and all claims, damages, liabilities, or causes of action arising out of or related to services received, including those allegedly caused by negligence.
SECTION 5: NO GUARANTEE / NO REFUND POLICY
I understand that due to the educational nature of student services:
- Results are not guaranteed
- No refunds are provided
- Service adjustments may be offered at instructor discretion
SOCIAL MEDIA CONSENT
Ascend Beauty College may take photographs or videos for educational or promotional purposes.

I understand that once content is published online, it may be shared or reposted by others beyond the School’s control.
